Notice: Currently limited availability and near full capacity with clients. If no appointments are available, please check back in future weeks. If you are a student, athlete*( sports, dance, gymnastics, etc), or an individual with a diagnosed eating disorder and you are not able to schedule with the booking links below, feel free to email me at stacieellisrdn@yahoo.com to see if I have current availability in the evening. Evening appointments are reserved for these special populations and are not going to be listed on booking link.
*Athletes that can be currently seen include individuals who currently practice 3 or more days a week in a particular sport, dance, gymnastics, martial arts, etc. It does not include body building, individuals going to the gym for weight loss or muscle gain purposes, or novice athletes that are training for their sport 2 days a week or less. For those individuals, please refer to the regular booking link

Please note that it is the client's responsibility to inquire about their Nutrition Counseling benefits with their insurance. To find out, you can call the customer service number on the back of your card. Clients will be responsible for any charges not covered by insurance.

All consultations are conducted on either online via Zoom or in person at 610 Uptown Blvd, 2 Suite 4200, Cedar Hill, TX, or at the University of Texas at Arlington Maverick Activity Center. Students from the University of Texas at Arlington may meet in-person at the Maverick Activity Center, my office in Cedar Hill, or via TEAMS UTA. Students please go to the Campus Rec website for your nutrition inquiries.
Initial Nutrition Assessment- Includes a1 1/2 hour nutrition assessment. The assessment involves looking at your medical history, diet practices, and lifestyle habits. After the assessment we will work on developing goals and the best nutrition care plan that will work for you to meet your goals.
Follow Ups- Each follow up exam is 30-60 min depending on your current nutrition needs. There are many layers to nutrition, and it is impossible to cover everything in one appointment. It takes time to make new dietary lifestyle habits; therefore it is highly recommended that clients schedule follow up appointments in order to gain the most benefit from their nutritional services. Dietitians can act as an accountability partner and help guide you on your way. Research shows that individuals who sees their dietitian at least 1x per month for 6 months are more likely to maintain their dietary changes, than individuals who go fewer times.
Before your Consultation
You will be required to fill out the Health Assessment on the Healthie app or on paper form. You can find the HIPPA information here. If you have any current labs, medication lists, or medical records that are pertinent to consultation please have them available at the time of the appointment. You may also upload them to the Healthie app.
Insurance: Excellis Nutrition currently accepts only Blue Cross Blue Shield, Aetna, Cigna, Golden Rule, Optum, Oxford, Select Health, United Healthcare, United Medical Resources (UMR) insurances for her practice.
Cancellations
Dr. Ellis is committed to helping individuals reach their health and nutrition goals. In order to provide consistent care and maintain appointment availability for all patients, Her cancellation policy to require 72 hours’ notice prior to an appointment.
This policy applies to late cancellations, last-minute rescheduling, and no-shows.
She understands that unexpected circumstances happen. Emergency cancellations, such as unexpected illness, car accidents, or other similar circumstances outside of the patient's control, will be considered on an individual basis.
Cancellations made with less than 72 hours’ notice due to scheduling conflicts, work obligations, or other non-emergency circumstances will be considered late cancellations under this policy.
Cancellation Policy:

Patients are allowed up to 3 late cancellations within a 6-month period.

Patients are allowed up to 2 no-shows within a 6-month period.

Emergency cancellations that are approved by Dr. Ellis will not count toward these limits.

After reaching the cancellation/no-show limit, services may be paused.

Patients may request to resume services after 6 months, subject to appointment availability.
A $75 cancellation fee will apply to each late cancellation and no-show that does not qualify as an emergency exception. If a cancellation fee applies, you will receive an invoice for the $75 fee. Unpaid cancellation fees are subject to the payment and collection terms outlined in the patient financial agreement.
Appointments are reserved specifically for each patient. When an appointment is cancelled with less than 72 hours’ notice, it is often difficult to offer that appointment time to another patient. This policy helps us maintain appointment availability and provide fair access to care for everyone seeking nutrition services.

Refunds
Refunds are not available for clients after they receive a service. If a client must leave the appointment early, the client will only get charged for the time they used. Note, if a client is late, they will be billed from their scheduled start time until the end of the session.
