Excellis Nutrition currently only accepts Blue Cross Blue Shield of Texas insurance. Those with BCBS are asked to provide Dr. Ellis with a copy of their insurance card prior to their appointment so that benefits can be verified and an appropriate nutrition care plan can be determined to meet your needs.
Those who are covered under another insurance, Dr. Ellis can provide a super bill that you can submit to your insurance company for reimbursement if your insurance allows it for an option. Excellis Nutrition understands this being an inconvenience for some individuals, and Excellis Nutrition is willing to work with you to determine the best package to meet your budget needs. Special discounts are also available throughout the year and to certain populations including active and retired military and spouses (military ID required), first responders (police, firefighters, EMT, Paramedics), and college students (enrollment verification needed). Cancellation Fees
There will be $25 cancellation fee for any appointment where a client does not show up and they failed to cancel within 24 hours of their appointment in order to cover the administrative charges that are in conjunction with your appointment. Upon scheduling your appointment you are required to agree to paying this fee if you fail to cancel in sufficient time.
Refunds
All services provided by Excellis Nutrition are non-refundable.
Why the change in operating hours in April?
Excellis Nutrition LLC is excited to announce that they are partnering with another agency to take care of the insurance side of the business. As a result, there are deadline requirements for submitting claims. Due to this, Excellis Nutrition believes the best approach is to take care of the claims at the end of each working day which is why the limit on the number of hours per day. In addition, due to the company taking care of some of the work, Excellis Nutrition now can provide more days for appointments, and Excellis Nutrition will become insured with all the major insurance companies, allowing for more people to get the nutrition help they need. It is a small trade off for the benefit of reaching more people in need of nutrition help.
