Insurance Information and Cost

Excellis Nutrition currently only accepts Aetna, Anthem, Blue Care Network, Blue Cross, Blue Cross Blue Shield, Blue Shield, Carefirst, Cash Pay, Cigna, Empire, Golden Rule, Highmark, Horizon, Independence Blue Cross, Optum, Oxford, Regence Blue Cross Blue Shield, United Healthcare, United Medical Resources (UMR). It is the patient's responsibility to ensure their insurance covers nutrition counseling.  Patients can do this by calling the customer service line on the back of their card and asking the benefits for nutritional counseling.  This should be under preventative care. Patients will be responsible for any costs not covered by their insurance, and an invoice will be sent to the client if anything is not covered. 

 

Those who are covered under another insurance, Dr. Ellis can provide a super bill that you can submit to your insurance company for reimbursement if your insurance allows it for an option.  Excellis Nutrition understands this  being an inconvenience for some individuals,  and Excellis Nutrition is willing to work with you to determine the best package to meet your budget needs. Please contact Excellis Nutrition for Dr. Ellis's pricing, you can do so by filling out the contact form here and she will email you back in 24-48hours, or consider scheduling a Discovery call if you would like to speak with her regarding the services and the different options available.  Special discounts are also available throughout the year and to certain populations including active and retired military and spouses (military ID required), first responders (police, firefighters, EMT, Paramedics), and college students (enrollment verification needed).  Again contact Excellis Nutrition for more!

Cancellation Fees

There will be $60 cancellation fee for any appointment where a client does not show up and they failed to cancel within 24 hours of their appointment in order to cover the administrative charges that are in conjunction with your appointment.  Upon scheduling your appointment you are required to agree to paying this fee if you fail to cancel  in sufficient time.  

Refunds

All services provided by Excellis Nutrition are non-refundable.
